Baptist Care SA’s Living Connected, Living Well (LCLW) program will strengthen social inclusion in Eastern, Northern, and Southern Adelaide by supporting vulnerable people aged 18-64 (or 49 years for Aboriginal people) living in social isolation to engage with, and participate in, community activities, groups, and volunteering opportunities.   People with an NDIS or Aged Care plan are not eligible for this program.

Baptist Care SA has formed unique consortia in each region consisting of organisations fully entrenched in each region with long histories of specialised local service excellence. From day one, each consortium will deliver LCLW from a firm base of well-established local community networks, links and relationships. The service will be implemented and delivered by organisations known and trusted by the local community.

In establishing each consortium, Baptist Care SA has strategically partnered with organisations whose specialised skillsets are perfectly suited for the delivery of different LCLW service model components.  Consortium partners, generally speaking, fall into one of two categories.

  • Local Community Centres and/or Volunteering Agencies
  • Primary and Secondary Health Providers

The partnership model enables LCLW to provide clients with holistic, wrap-around support and seamless transition pathways between agencies.

The ultimate goal for clients of the program will be to transition from social isolation to community participation.
