Baptist Care SA launches new Disability Pathways Service Delivery Model

 In line with Baptist Care SA’s Strategic Horizons Plan, we have an ongoing commitment to fostering safe, inclusive practice and continuous improvement.

To ensure that our clients feel safe, comfortable and have confidence that they will experience the very best service with us, our Disability Pathways team has been proactive in developing a Disability Pathways Service Delivery Model.  This aligns our services and practices with the NDIS Practice Standards and Rules and underpins the quality and safety of our client supports.

Our Disability Pathways Service Delivery Model also ensures that our staff are clear about our expectations of them and empowers them to deliver to the highest standards with a genuinely client-centred approach.

 

Our Disability Pathways Service Delivery Model offers: 

  • complete, thorough and accurate support planning which is inclusive of the client and their chosen support network
  • active and positive behaviour support, focussed on strengths and capacity building
  • support that is underpinned by human rights and choice and control
  • regular feedback from our clients and their chosen support network
  • an environment in which clients and their chosen support network have a voice
  • connections and positive relationships with community
  • flexibility around support needs and circumstances.

Outcomes include: 

  • client-centred practice where a person’s fundamental human rights are upheld
  • a quality client experience
  • the safeguarding of vulnerable people
  • adaptive and flexible client supports
  • demonstrable quality practice where outcomes can be achieved and measured
  • clarity in roles and expectations across staff levels
  • increased staff support and improved performance
  • continuous quality improvement.
