How we can help you 

Community Connections supports people to establish and maintain meaningful social connections with community, supportive social networks and a sense of belonging.

It supports people aged 18 to 64 who are not eligible for mainstream programs, such as NDIS and My Aged Care, to be more involved in their community and connect to support networks and services.

Priority is given to people who are unpaid carers, Aboriginal and Torres Strait Islander peoples, from new and emerging culturally and linguistically diverse communities, financially disadvantaged, living in communities of persistent or location-based disadvantage or identify as being from the LGBTIQA+ community.

The Community Connections program can be for up to 12 weeks, however we can connect people with longer-term support, if needed.
