Tina

Tina*, 40 years old, female, Aboriginal

We received a referral for Tina* from Sonder after Tina’s NDIS application was rejected. Tina lives alone and has a long history of mental ill-health, with a diagnosis of depression and PTSD. Tina also lives with chronic back pain, Tourette’s syndrome and epilepsy which is managed by medication.

Tina spends the majority of time inside her home and does not have many friends/family that can support her. Tina has an interest in art and animals and would like to start engaging with her local community more to build a network of friends, but also to have things to do during the week.

Tina is continuing to be supported by Sonder for mental and other health conditions, and also has 2 hours per week of support for practical support in the home and grocery shopping etc.

Tina and her Community Connections case manager are looking to link to a Tourette’s syndrome support group and at local community centres that offer art-based programs.

Mary

Mary*, 53 years old, female

We received a referral for Mary* from Carer and Community Support, with who she is linked as she is the primary carer for her son who has a disability & complex needs.

Carer and Community Support identified that Mary spends the majority of her time in the home and currently only attends a carer’s support group once per week.

Mary has expressed wanting to get some support in the home, which her Community Connections case manager will make a referral for.

Mary’s Case Manager will also help in identifying her personal interests/goals through the use of the case plan and linking her to relevant supports/programs that align with those goals.
