Sometimes people who are feeling isolated, struggling with their mental health or living with a disability can experience feelings of loneliness and disconnection.
If this is you or someone you know, we can help.
Our Community Links Program (CLP) provides one-on-one support to people aged 65 over, or 50 and over for Aboriginal and Torres Strait Islanders persons, both at home and in the community.
Together, we can help you reconnect with social and recreational activities in your community to increase your confidence and independence.
