Connecting you with your community

 

Sometimes people who are feeling isolated, struggling with their mental health or living with a disability can experience feelings of loneliness and disconnection.

If this is you or someone you know, we can help.

 

Our Community Links Program (CLP) provides one-on-one support to people aged 65 over, or 50 and over for Aboriginal and Torres Strait Islanders persons, both at home and in the community.

 

Together, we can help you reconnect with social and recreational activities in your community to increase your confidence and independence.

 

We start by helping you set up your goals. Some of these might include…

  • Identifying your interests and hobbies
  • Developing your social skills
  • Taking public transport

 

We can also:

  • Support you to attend a local group/activity
  • Provide companionship and visit you at home
  • Help you with your shopping
  • Support you to attend appointments
  • Provide a friend on the phone
