Daily Living

It is important to us to know the real you. This makes a big difference to the quality of support to assist you with personal tasks of daily life and to maximise your independence and freedom.

 

Home

We can assist you to improve your living arrangements and support you to choose, maintain or find accommodation.

Health & Wellbeing

Staying fit and healthy is really important and if your goals include learning how to balance a healthy diet and exercise into everyday life, we’d love to inspire you to go for it!

Lifelong Learning

It’s a significant milestone when you leave school and go on to further education or work. We can support you to make this transition a really great experience.

  • Support to transition through school to further education or employment.

 

Work (pre-employment preparation)

Getting ready to work or volunteer involves preparation. We can support you to feel more confident by teaching you some of the skills needed to develop your independence. We’ll find out from you what you need to work on and then together in a supportive environment, we’ll get started.

Social & Community Participation

If you’d like to be more active in your community, we’d love to support you to get more connected. Depending on your interests and need for support, we can work out together what you like to do, where you like to go and find out more about who you can do it with.

What happens when you contact us...

1. Welcome
When you contact us, you will be greeted by our Disability Care Team. You can ask them any questions or just have a chat with them about your needs. They are always willing to help

2. FREE Initial Meeting
Next, we would like to meet with you in your home, or another relaxing and comfortable environment. There is no cost for this initial meeting and we enjoy getting to know about you and your hopes and dreams

3. Support Plan
We will sit down together and discuss your needs and goals. We’ll agree on how we can work together. Then we will create a tailored and personalised plan for your care

4. Meet the Team
We want you to be comfortable with the people who work with you, so you will have the opportunity to meet the person or people supporting you

5. Review
We will set regular review meetings so we can check in together and make sure everyone is happy with how things are going

What is the NDIS?

The National Disability Insurance Scheme is a new way of giving greater control and choice to people living with a disability, their families and carers. This means that funding now goes to you and your family so that you can choose what disability supports you receive, when you receive them and who delivers them.

The new insurance scheme focuses on you, your goals and dreams. It’s a huge change to the process of disability funding and it is being rolled out across South Australia now.

 

If you are an Australian under the age of 65 with a permanent or significant disability, you may be eligible for NDIS funding.
