Specialist Support Coordination

We understand that your situation may be complex and we are here to support you. Our Specialist Support Coordinators are on your team every step of the way, helping you to manage challenges in your support environment and ensure consistent delivery of service.

 

Our Specialised Support Coordinators offer a higher level of Support Coordination. We work directly with clients who often have complex needs and may be in a crisis situation. By working with you and those close to you, we assist in contacting services and providers to navigate crisis and beyond.  These services may include allied health, community organisations, advocacy supports and government agencies.

  

With approved NDIS funding, our Specialist Support Services can also assist with an NDIS service called Exploring Housing Options (EHO). EHO is a support funded in an NDIS plan for people who are ready to access supported accommodation. A Specialist Support Coordinator will work with you to:

 

  • arrange allied health assessments to determine your support needs or specialist housing design features
  • assist you to define your housing preferences
  • help you look for appropriate housing
  • write your EHO Housing Plan.

 

We can also support you in preparing for your plan review meeting, to seek additional funding or to obtain allied health reports.
