Connecting you with your community

Sometimes people who are aged, living with a disability or with mental health issues experience feelings of loneliness and isolation. If this is you, or someone you know, we can help.
 

Our Community Links team supports adults living in the Southern Metropolitan region of Adelaide to connect with others, to thrive in their community.
We offer three different programs:

 

Social-ize Telephone Support

A friend on the phone – If you’re feeling a bit isolated, we can arrange for one of our friendly staff to call you regularly to have a chat and keep in touch.

Disability Social Support

Fun things to do and great people to meet – If you’re living with a disability or experiencing mental health issues, we can provide one on one support either at home or in the community.  We’ll help you to regain your independence, establish meaningful relationships and enjoy social and recreational activities close to home.

MyPace Support

Increase your sense of wellbeing – If you’re over 65, or if you’re over 50 and identify as an Aboriginal or Torres Strait Islander, we can provide one on one support at home or in the community. Tailored to your individual needs, we can help you to maintain your independence.
Entry to the MyPace Program is made by contacting My Aged Care on 1800 200 422.
