Taking steps to link with your community and support your wellbeing to help you make the most of life.
Sometimes people who are feeling isolated, struggling with their mental health or living with disability can experience feelings of loneliness and disconnection.
If this is you or someone you know, we can help.
Our Community Links Program (CLP) provides one-on-one support to people aged below 65, or under 50 for Aboriginal and Torres Strait Islanders persons, both at home and in the community.
Together, we can help you reconnect with social and recreational activities in your community to increase your confidence and independence.
We start by helping you set up your goals. Some of these might include…
We can also:
“The Community Links program really helps you get back into the community again.”
“It has helped me feel more confident.”
“I always used to be on my own, now I go to activities and I enjoy being around other people.”
